BACK PAIN - Health Information

Medications

Medications are normally used to alleviate muscle spasms, inflammation, pain and disrupted sleep, but will not cure back pain associated with disk degeneration. Two categories of back pain medications that are available include: the one’s that alleviates the symptoms, and the one’s that can help alter the disease progression itself.

Medications That Control Symptoms:

Acetaminophen

Acetaminophen (Exdol®, Panadol®, Tylenol®, and others) works as a painkiller and is often prescribed to give mild to modest back pain relief. The drug acetaminophen does not classify as an anti-inflammatory and can more often than not be safely taken with an anti-inflammatory drug to relieve pain.

Acetaminophen may bring temporary back pain relief and lower back pain relief; it will not facilitate the original origin of the condition. Overindulge of drugs can create liver damage and stress to the organs. NSAIDs

COX-2 Inhibitors

 

These are a subclass of NSAID’s that have more recently been introduced to North America and include the following: Rofecoxib (Vioxx®), Celecoxib (Celebrex®), Meloxicam (Mobicox®), and Valdecoxib (Bextra®). Standard NSAID’s inhibit proper blood clotting whereas Cox-2 inhibitors do not but DO share the negative side effects, which include: kidney failure, heart attack and numerous cardiovascular complications.

Vioxx®, the cox-2 inhibitor made by Merck, has been pulled from the market because of severe lethal side effects due to heart attack and stroke. This particular drug’s ability to provide relief for back pain has been under scrutiny therefore alternative back pain relief is recommended.

Cortisone

 

  • Injection of cortisone provides almost immediate back pain relief for inflamed, swollen or tender joints.
  • Injected directly into the joint in cases of severe inflammation and swelling.

Lift objects wisely

Carry objects against your body

When you hold or carry heavy objects away from you, your body is susceptible to back injury. Heavy objects should be held close to your body when lifting them. Use caution with your golf swing

Be careful when exiting vehicles

When you exit your vehicle, turn your entire body to the side. Position both feet on the ground then stand up carefully. When you enter a vehicle, reverse this procedure. First sit down, then swing both legs in at the same time, this will not twist your back.

Sit correctly

The human spine was not designed to sustain a seated position for extended periods of time.  Sitting at the computer, long drives, watching television or being seated on a plane are some activities that can create back pain. The most effective way to avoid back strain is to get up and walk around every fifteen minutes. When you are driving for long periods of time, pull over whenever possible, and stretch your legs.

When sitting for long periods of time at work, try to sit in a chair that is well designed. One that supports your back, and permits you to change your position easily is recommended. While driving, tuck a cushion behind your back. This will provide some support and be more comfortable.

Stay fit

Exercising on a regular basis will help you avoid any strains or stresses that can result from weakened back muscles. Low-impact aerobics such as walking and swimming will help strengthen the muscles in the abdomen and the back area. There are fitness organizations and recreation centers that offer exercise programs specially designed to manage back pain.

Cope with stress

Muscle Relaxants

Muscle relaxants that address back pain are available in either an over-the-counter medication or as a prescription medication. Prescription medications comprise of Flexeril® (cyclobenzaprine), Soma® (carisoprodol), and Valium® (diazepam). Soma can potentially be habit-forming; therefore it is typically only prescribed as a temporary solution for back pain. Concerns arise if it is combined with alcohol and/or drugs that may have a psychological effect. Long-term use of Flexeril appears to be more acceptable. This drug has the tendency to impair physical, and mental functions. Men who have enlarged prostates can develop urinary retention. Cortisone

An epidural steroid injection (ESI) maybe recommended when analgesics are ineffective. This procedure is exercised by injecting small amounts of cortisone directly into the spinal canal. A powerful anti-inflammatory drug such as cortisone can help reduce the inflammation that surrounds the nerves. Unfortunately, ESIs are only successful in 40 - 50 percent of cases; therefore little pain relief is actually achieved.

Surgery

Surgery is typically used as a last option for back pain management. All surgeries are unique to the individual’s particular symptoms. No procedure is the same, and no one type of operation will be effective for everyone. There are three surgical procedures that are commonly performed for back pain; these include: spinal fusion, diskectomy, and complete laminectomy.

Diskectomy - removal of a herniated disk to alleviate any pressure on the nerve root.

Laminectomy - removal of the lamina (the back section of the spine that structures the top of the spinal canal) to allow more space for the nerves.

Spinal Fusion - insertion of a bone graft between two or more vertebrae. This causes them to fuse together, halting any motion in between the vertebra, which is a result of segmental instability.
