What is Gout?

Gout is a type of arthritis that causes intense swelling and pain that can appear overnight. It mainly affects older males and symptoms manifest in a similar pattern as its arthritic counterpart. How Common Is Gout?

Medications For Gout

Your doctor will administer certain tests, which will most likely include a 24-hour urine sample. He will then determine which medication is best suited for your condition. Some individuals simply do not eliminate adequate uric acid from the body, while others tend to overproduce uric acid. There are different medications, which are used for each circumstance. Medications That Control Gout Symptoms

Medications most often include anti-inflammatory (NSAIDs) and non-steroidal drugs that help reduce the amount of pain you experience. These drugs do not inhibit the process of the disease. Consequently, they do not prevent bone or joint impairment or damage, which is common in advanced cases of gout. This approach can typically take anywhere from several days to several weeks before the pain is under control.

NSAIDs are pronounced "ensayds". This category of drugs help manage pain, reduce inflammation in the joints and decreases stiffness. Lower doses can help decrease the pain, but in order to reduce inflammation, higher doses are necessary. These are often recommended for symptoms, but unless the causes of gout are eliminated, the problem may persist. A gout diet is advised in conjunction with this and all gout treatments.

The problem with NSAIDs is their side effects.

  • Taking multiple NSAIDs at one time can increase the risks of developing heartburn.
  • Inhibits the blood clotting properties in the blood and can interfere with blood-thinners such as coumadin.
  • NSAIDs can cause serious adverse affects such as bleeding and ulcers.
  • Kidney disease has been a reported side affect.

Prescriptions are required for most NSAIDs. These include: naproxen (Naprosyn®), nabumetone (Relafen®), indomethacin (Indocid®), diclofenac (Voltaren®), piroxicam (Feldene®) and sulindac (Clinoril®). There are over-the-counter NSAIDs available such as ASA (Aspirin®, Anacin®, etc), and ibuprofen (Advil®, Motrin IB®, etc) that do not require a prescription.

COX-2 Inhibitors. This class of NSAIDs is the most commonly used Cox-2 inhibitors in North America. These include rofecoxib (Vioxx®), celecoxib (Celebrex®), valdecoxib (Bextra®) and meloxicam (Mobicox®). Cox-2 inhibitors do not interfere with adequate blood clotting like standard NSAIDs. However, recent studies strongly confirm that Cox-2 inhibitors and standard NSAIDs have the same adverse affects. These drugs may contribute to diseases such as kidney failure. There is also an increased risk of cardiovascular problems or heart attack.

Acetaminophen (Tylenol®, Panadol®, Exdol®, and others)

  • Can safely be combined with anti-inflammatory drugs.

The most common, although rare side effect is a rash, and can be followed by a severe allergic reaction. If you experience a fever, chills, rash or joint pain, discontinue use of this medication and call your doctor. Nausea, vomiting and diarrhea are also common side effects of both drugs.

What Are Your Gout Risk Factors?

  • If you have certain degenerative conditions that cause excessive breakdown of the cells. This can raise serum purine levels, which increases the amount of uric acid within the body.
  • If there are genetic factors. An estimated one in four individuals with gout has a hereditary predisposition for the disease. In certain families, there is an absence of the enzyme, which helps break down purines in the body.
  • Excessive eating. Individuals, who over eat, often develop gout since large quantities of food tend to increase the level of uric acid produced by the body.
  • If you consume a diet that is high in purine. Foods that are rich in purine break down in the body and create uric acid. Foods high in purine include trout, herring, and mackerel and organ meats. Peas and beans contain purine, but at reduced levels.
  • Certain medications. There are some drugs that decrease the body’s ability to eliminate uric acid. Salicylates (Aspirin®, ASA), diuretics, (levodopa (Prolopa®) and cyclosporine (Neoral®) can play a part in the development of gout. Excessive use of niacin (vitamin B3) can also contribute to the condition, since it raises uric acid levels.

Acute symptoms of gout (flare-ups) seem to happen more frequently under certain conditions. These include:

  • If you have had an infection
  • If you have been hospitalized or have had surgery
  • If you have experienced a joint injury
  • If you consume excessive alcohol or foods rich in purine
  • If you are dehydrated

Gout Wellness Tips

The following tips will help you manage gout more effectively:

  • Avoid foods rich in purine from your diet. Most animal products are high in purine, with the exception of eggs and low fat dairy. Animal products include organ meats (kidneys, liver, heart, sweetbreads); mackerel, herring, trout and anchovies, which are especially high sources of purines. You should also try to avoided gravies and broths. Peas and beans contain minimal amounts of purine, but should still be limited to 1or 2 servings per day. Peanuts are considered to be a legume, but its purine level is minimal, with less than 50 mg per 100 grams. Legumes such as peas, beans and lentils contain between 50 and 150 mg of purine per 100 grams.
  • Maintain a healthy weight. Try to avoid rapid or sudden weight loss and crash diets, which can initiate severe flare-ups.
  • Increase your daily consumption of fluid, which will help reduce uric acid in the urine. This can also help prevent the development of kidney stones. Drink between 8 and 12 cups (2 or 3 litres) of liquid every day. Your daily fluid intake can consist of water, vegetable juice, unsweetened fruit juices, herbal teas, soups and milk.
  • Reduce your caffeine and alcohol consumption. All alcohol (especially beer) contains purines. Alcohol also interferes with the excretion of uric acid in the body. Excessive drinking increases blood triglycerides and adds unnecessary weight, which can initiate the onset of gout. A moderate intake of alcohol such as one drink per day will not likely increase the risk of a gout attack. Talk to a professional about alcohol consumption.
  • Consider taking a vitamin B12 supplement. If you take colchicine, a medication for gout, consider adding vitamin B12 to your diet, since colchicine interferes with the body’s absorption of B12. Excellent sources of vitamin B12 are found in animal foods such as poultry, meat, and fortified rice or soy beverages. Eggs and dairy products are also good sources of vitamin B12, but should be limited since they are rich in purine.
  • Eat plenty of black cherries. New studies suggest that high consumption of black cherries can benefit individuals who suffer from gout. There are certain plant compounds found in cherries (anthocyanins) that can help prevent gout.
  • You can help reduce inflammation by adding more raw seeds such as flax and pumpkin seeds or fish oil supplements into your diet. These are health-promoting foods that are rich in omega 3 fatty acids, and can reduce inflammation in the joints. Increase your daily amount of fresh vegetables and fruit. Reduce foods that contain trans fats (hydrogenated oils) and animal fats (arachidonic acid), especially fatty meats such as beef or pork including dairy products with high-fat contents.Celery seeds are considered a food-based remedy that has been proven to help prevent gout flare-ups.
